Below you will find a chart detailing the cash benefits you will receive using your Hospital Indemnity Insurance, based on your membership level. These cash benefits are the amount you will be paid per day for each accident or sickness occurrence. You will receive these cash payouts based on your membership level until you hit your annual max of occurrences per benefit, or your overall annual max benefit amount if applicable.

  1. Hospital Max Annual Benefit represents the maximum amount which you can incur for Hospital Confinement benefits in a given year, and only applies to states insured by the Crum & Forster policy (there is no separate Annual Max for states insured by the Met Life policy).
  2. For states insured by Crum & Forster, all Benefits are per day. For states insured by Met Life, Hospital Confinement benefits are per day. Emergency Room and Dr Visits benefits are per visit, Ambulance benefits are per trip, Surgery benefits are per procedure, and Diagnostic Procedures are per test.
  3. Doctor visits include primary, specialist, and urgent care physicians. For States insured by Met Life, there is a per family maximum of 8 Doctor visits per year, in addition to the maximum of 4 per person.
  4. Limit 7 days per confinement. Annual Max applies to states insured by the Crum & Forster policy (there is no separate Annual Max for states insured by the Met Life policy).
  5. Surgery benefit includes 2 in-patient and 2 out-patient procedures for a total combined allowance of 4 covered surgeries.
  6. For states insured by Met Life, Air Ambulance is covered at a higher benefit amount which can be found in the certificate of coverage.
  7. For states insured by Met Life, Laboratory Tests are covered under the Diagnostic Procedure benefit, and there is a maximum of 2 per policy year which can be used for either imaging or laboratory tests.
